Nursing Home Transition is a priority at DON, provided to individuals residing in nursing facilities who choose to live independently. This service assists people with disabilities to find accessible housing, transportation, personal assistance services and other community resources necessary for full inclusion into society.
Nursing Home Transition is a comprehensive coordination of services and resources to provide alternative living arrangements to residing in a nursing facility or other institutional setting. Steps include:
- Determine if you meet qualifications for safe and healthy transition
- Act as a liaison between facility and you the consumer
- Assist in developing an integrated plan that meets the individuals needs
- Assist in finding and securing housing (including needed household goods)
- Provide community resource information
- Coordinate date of discharge
